Requesting a Fuel Pin

There have been a few requests lately on how to get team members assigned a fuel pin. The process is detailed below. Please note that very recently Fleet Services updated the diesel fuel pump to be controlled by the fuel station computer. You must now have the fuel card and a PIN to refuel the F350.

Effective June 2015, Georgia Tech Fleet Services has implemented a new Fuel Card Program, which requires all current and potential PIN users to complete a Fuel PIN packet. The packet consist of:

  1. Fuel Card Agreement & PIN Request Form (here), the required People Soft ID # is DE00005340, the department should be ME department regardless of major, print this form out and complete the actual signature on the paper in pen. Complete the first section and once you get to the Departmental Approval section, only sign and date for Pin Requester, and leave the rest the blank.

  2. Class C Operator Training Certificate - Read through this presentation, and print out the last page. Answer the quiz, print your name and sign and date under Pin Requestor.

  3. Motor Vehicle Driver Use Acknowledgment - Print out this document, and complete all sections. Reminder that your department is the ME Department for these documents.

  4. Fuel Card Certification Tutorial - Read through this presentation, and print out the last page. Answer the quiz, print your name, and sign and date under Pin Requestor.

  5. DOAS Fuel Card Acknowledgement - https://service.doas.ga.gov/app/answers/detailopa/a_id/1587 - Complete this fuel card acknowledge, and print out the certificate that comes with it.

Once you have completed the trainings and printed and filled out the five documents, please bring all the paperwork in-person to the appropriate supervising ME faculty member (currently Gary Barkley) to submit. He will need to sign documents 1 , 2, and 4.

The current POC is Gary "Bruce" Barkley (gary.barkley@me.gatech.edu) in ME Facilities. They have to sign the request form as your "supervisor"; you can not take this packet straight to Fleet Services.

Once you have the full packet, you cannot submit via e-mail and must submit one of the following ways:
Forms and certificates must be sent one of the following ways:

  1. In-person delivery to the Office of Fleet Services

  2. Interdepartmental mail (Mail Code: 0350)

  3. Fax to 404-385-2401

  4. US Postal Service - Please send to 306 Tenth Street, NW, Atlanta, GA 30318
