SCC PAPR Registration Program
First step:
Complete SCC internal survey: https://forms.gle/oWUafh49xRDAkizz9
Second step:
Complete, print and sign both Concentra authorization and evaluation forms:
email the completed packet individually to midtown@concentra.com
the review may take up to 72 hours of more
a member from GT EH&S will contact you stating your results
The results will be that you are medically cleared to wear a respirator, or you are not medically cleared
team leads email the list of those who have sent the packet to Concentra to Alicia Wood-Jones

Fourth Step:
Alicia Wood-Jones (EHS Lab/Chem Team) will email each student who has been medically cleared for PAPR use.
IF STUDENT IS NOT MEDICALLY CLEARED: Alicia Wood-Jones will email student directly. Student will then be asked to schedule an appointment with STAMPS for a physical. Student will need to pick up paperwork from Alicia Wood-Jones prior to appointment with STAMPS. If student is NOT cleared, student will not be allowed to participate in the Respiratory Protection Program at the SCC.
Jake Tompkins will grant you authorization to the tool through SUMS
Fifth Step:
Take boxed respirator set from shelf and charged battery from chargers
Scan QR code on box to check out PAPR in SUMS before use
During use, keep track of time and type of usage
When finished, remove battery from unit and place into an empty charging port
Ensure that hood, filter(s), tube, and pack are returned to storage box
Fill out form with usage time and other requested information
You must check out the PAPR to use via SUMS

Sixth Step:
Take great care of the PAPRs as these are expensive pieces of safety equipment meant to be shared by all teams and their members for a long time to come.
